BBL in Turkey: A Safety-First Guide to Buttock Augmentation
By the Lamis Clinic Plastic Surgery Team, Istanbul, with insights from Dr. İsmail Barut, Plastic Surgeon · Updated October 2026
Short answer: buttock augmentation can be done with your own fat (a BBL), with implants, or with both. A BBL can be done safely, but it has historically carried more risk than most cosmetic operations — so the surgeon’s technique matters far more than the price. Plan on a longer stay than most procedures, and on about two weeks without sitting directly on the area.
Fat transfer, implants or both
“Buttock augmentation can be performed using either fat transfer (BBL) or implants, and combining both techniques is also possible.” — Dr. İsmail Barut, Plastic Surgeon
BBL (fat transfer): fat is taken by liposuction from areas such as the tummy, flanks or back, purified, and used to shape the buttocks and hips. No implant is left behind.
Buttock implants: a soft implant adds volume, useful for slimmer patients without enough spare fat.
Combined: an implant for volume and fat for shape.
Why BBL got a dangerous reputation — and what changed
The main danger is a fat embolism: fat injected into or under the buttock muscle can enter large veins and travel to the lungs. In 2017, plastic surgery societies formed a task force after finding BBL deaths were much higher than for other cosmetic operations. Their guidance: inject fat only into the layer under the skin, never into or under the muscle, and angle the instruments away from deep veins. Some places now also require ultrasound so the surgeon can see exactly where the fat goes.
Some older reports put BBL deaths as high as about 1 in 3,000 operations. More recent surveys suggest the risk has fallen a lot — to about 1 in 14,000 — where surgeons follow the safer technique.
The one question to ask any surgeon
“Where exactly do you place the fat?” The answer you want: only in the fat layer under the skin — never in or under the muscle. If a surgeon can’t answer clearly, look elsewhere.
How many BBLs do you perform, and how many per day?
Do you use ultrasound to guide the injections?
Where will the operation take place, and who gives the anaesthetic?
How much fat is safe to transfer for my body?

How long can’t you sit after a BBL?
“If the procedure is done with fat transfer, we generally recommend avoiding sitting directly on the area for about two weeks, or using a special cushion when sitting. If implants are used, follow precautions for at least one month.” — Dr. İsmail Barut
How long to stay in Istanbul
For longer operations on several body areas — which often includes BBL — the British Association of Aesthetic Plastic Surgeons advises arriving a week before surgery and staying 2–4 weeks after. That leaves time for check-ups and lowers the risk of blood clots on the flight home.
Recovery, step by step
Days 1–7: soreness and swelling; compression garment day and night; short walks several times a day
Weeks 1–2: no sitting directly on the buttocks — use a special cushion under your thighs
Weeks 3–8: gradual return to normal activities and exercise as your surgeon allows
Months 3–6: some transferred fat is naturally absorbed and the final shape settles
Who is not a good candidate
People without enough spare fat — implants may suit them better
Smokers who can’t stop before and after surgery
People with a history of blood clots or certain medical conditions
Questions patients ask us
How long do BBL results last? The fat that survives the first months behaves like your own fat — it stays, but changes if your weight changes.
Can I fly home a few days after a BBL? We don’t recommend it. Long flights soon after this surgery raise the risk of blood clots.
